Hospice RN Case Manager

Coordinate individualized hospice care by assessing patients, collaborating with the interdisciplinary care team, and developing care plans. Maintain accurate patient records, monitor pain and symptom management, teach patients and caregivers, and communicate regularly with the clinical manager and care team.
