Job summary This is part of Acute Care at Home Service (ACAHS) which is responsible for delivering in the Community 2hr urgent care response via Community Rapid Intervention Service (CRIS), Virtual Wards and Acute Visiting Service. Ultimate aim is admission avoidance and early supported discharge to enable patients that would benefit from acute care within their own home.
